Change of Schedule Form

If you are doing a Preceptorship, Internship, or Independent Study in the program you will be manually enrolled so you will want to make sure to complete and attach this Change of Study form with the appropriate paperwork.

Note, if you are doing more than one in a semester, say a Preceptorship and an Independent study, you only need to complete one Change of Schedule form.
